SICK LEAVE POOL PLAN

Purpose of Plan:

Eligibility in the Pool

如果不符合任何资格要求,申请将被拒绝. No employee shall be unreasonably denied membership in the Pool.

Participation in the Pool

Participation in the Sick Leave Pool is voluntary and must be elected.  Eligible employees can apply for the Sick Leave Pool via workday:

  1. From the Workday home page, search for the  CREATE REQUEST
  2. Next to ‘Request Type’, type in “Sick Leave Pool Form” and click  OK.
  3. Fill out and answer ALL required fields, then hit  SUBMIT.

Upon completion of one year of Continuous Service at FAU, 符合条件的员工将在FAU的工作电子邮件中收到病假通知,通知他们的资格. 符合资格的员工必须在其资格之日起30个日历日内提出申请,以使其病假池会员资格于1月1日生效st  of the month following enrollment.

符合条件的员工将有另一次机会在年度开放注册期间加入病假池. 每年的开放注册期从10月开始,持续60天. 符合条件的员工将收到病假通知,其中包含开放注册日期和如何注册的说明.

会员申请被拒绝的员工可以在下一个开放注册期间重新申请. 必须满足病假池资格要求,以便员工在开放注册期间获得批准.

在符合条件的员工的病假池成员资格被批准后, 16小时未使用的病假从他们的账户中扣除并添加到池中.

Requesting Sick Leave Pool Hours

Participating employees who have depleted all of their accrued Sick, Annual, 和补休时间可以向病假池申请. 员工可通过填写“病假池工时”申请表,申请使用病假池工时   Application for Sick Leave Pool Hours  form and the Attending Physician's Statement form.

每次申请最多可获得160个病假小时(20个工作日). 参加计划的员工可在12个月期间获得最多480小时的病假(60个工作日). Sick Leave Pool hours will be pro-rated for part-time employees.

只有员工患有严重或灾难性疾病或受伤时,才可享受病假. 这将包括怀孕导致的严重或灾难性残疾, childbirth, or surgery. Uncomplicated pregnancy and recovery from childbirth is not included.

Returning to Work

当使用病假池时间的参与员工被证明能够返回工作岗位时, or otherwise assigned work-related duties, the payment of Sick Leave Pool hours will cease. Any unused Sick Leave Pools hours will be returned to the Pool.

如果员工在被证明可以返回工作岗位后使用病假池小时支付工资, or otherwise declared able to perform his/her assigned duties, 所有这些时间将从员工的个人病假账户中偿还给游泳池. 员工将不得使用其年病假时间,直到欠池的金额已偿还.

参与的员工有责任在事件发生后五天内通知员工关系部门他们有能力返回工作岗位或分配与工作相关的职责.

Termination from the Pool

在下列情况下,会员资格可被终止:

  1. 参加病假池的员工可以随时以书面形式通知病假池管理员/员工关系部取消其成员资格. 取消通知应包括取消的生效日期.
  2. Participating employees who retire, transfer, terminate, 或被终止在大学的雇佣关系,也将被终止在奖学金池中, effective on the date of the personnel action.
  3. 被发现滥用资源池的参与员工可能会被终止参与资源池.
  4. 如果员工从保险池中离职,并且欠保险池病假时间,那么这些时间将从他们的个人病假余额中扣除,如果这些时间是累计和可用的.

参与员工向保险池支付的任何病假将在员工取消保险池会员资格时被没收, retirement, resignation, or termination from the University.
